2. What data we collect from you?
Acibadem Sistina Hospital controls the personal data that you (as a personal data subject) give to us. Depending on your needs, we process the following types of personal data obtained from you:
- Identification data (name, surname, date of birth, personal identification number, clinical ID number, photograph, etc.)
- Health condition data
- Name and surname of the contact person
- ID card number
- Audio recording
- Video footage
- Contact data (address, e-mail, telephone and etc.)
3. How are your personal data collected?
You directly give us the personal data we collect. We collect and process the data when you:
- Come to the Hospital for a health service
- Reached our call center to make an appointment, to ask a question or to be informed about a certain service.
4. What purpose do we collect your personal data for?
We need your personal information in order to provide you with health services for hospital and outpatient examinations, in certain cases for which your consent is going to be obtained in advance to publish your personal data on media (newsletter, social media, billboards, brochures, web site, hospital TV and etc). Your personal data may be processed for other purposes, based on legal basis and for which we are going to make sure that you are additionally informed.
5. How your personal data are going to be used?
Your personal data, except from Acibadem Sistina Hospital, is processed by third parties with whom the Hospital has concluded an agreement. Hosting and storage of your personal data takes place in HIS (Hospital Information System), LIS (Laboratory Information System) as well as Tenay software whose base is located in Republic of North Macedonia.
In some cases provided by law, your personal information may also be disclosed to the Center for Public Health, the Ministry of Health and other state institutions acting in accordance with the Law.
When using third party services, Acibadem Sistina Hospital is going to ensure that the third party is going to provide appropriate technical and organizational steps regarding personal data protection, as required by the Law on Personal Data Protection.
6. Legal basis
The legal basis for the personal data processing depends on the processing purpose. Such as a certain law (Law on Population Protection from Infectious Diseases, Law on Health Care, Law on Patients’ Rights Protection etc.), agreement fulfillment between us and you as a patient, Acibadem Sistina business goals achievement or obtained consent.
7. How long are your personal information kept?
According to the Acibadem Sistina Hospital internal regulations, we are obliged to keep your personal data (data in medical history, examination data, etc.), in accordance with the Personal Data Protection Policy and in accordance with the Legal deadlines. After the expiration of the legally prescribed deadlines, your personal data are going to be permanently destroyed.
8. What are your rights?
Access right – you have the right to receive confirmation on whether your personal data is being processed, as well as to have access to your personal data and additional information in relation to personal data processing.
Right to Correction – If you believe that any personal data we collect is inaccurate or incomplete, you have the right to request and receive incorrect data or incomplete personal data supplementation.
Right to erase – you have the right to request in accordance with the legally prescribed deadlines for deletion of your personal data, and we will accept that request in case personal data is no longer needed to meet the purposes, in case you object to the processing and we fail to prove that our legitimate interest is greater than your fundamental freedoms and rights and in case personal data are processed illegally.
Right to transfer – you have the right to receive your personal data in a structured, commonly used, machine-readable format where you have the right to transfer that data to another controller. When exercising the transfer, you have the right to receive personal data direct transfer from one controller to another, if this is technically possible.
Right to restrict the personal data processing – you have the right to request the processing to stop while data accuracy is being determined, while checking if a legitimate interest prevails over your fundamental rights and freedoms and etc.
If you still think that your personal data is not processed in accordance with the Law, you can contact the Personal Data Protection Agency and submit a request to them.
